Summary RN Health Coach is a purposeful, result-oriented structured client interaction provided by an RN to promote the achievement of client goals. The RN Health Coach facilitates growth, healing, and wellbeing of the whole person using coaching principles and healing modalities that integrate body, mind, emotion, spirit, and environment. Health Coaching involves evidence-based conversations, clinical interventions, and strategies to safely engage clients with chronic conditions or at moderate to high risk of developing them. The focus is on disease management and education to reduce unnecessary hospital admissions by improving patient understanding and working closely with primary care physicians to detect exacerbations early, thereby avoiding ER visits. Responsibilities include: Completing RN Health Coach evaluations, including assessments of functional, psychosocial, safety, nutritional, medication reconciliation, cognitive, medical history, and Point of Care testing. Identifying client readiness to change and opportunities for growth. Establishing goals and outcomes with input from patients and caregivers, and implementing care plans. Evaluating and adjusting care plans as needed. Planning and structuring coaching interactions. Preparing progress notes and coordinating services timely. Following up with primary care providers and coordinating appointments. Communicating with PCPs and specialists regarding patient issues, therapy, and medication changes. Providing emotional and physical comfort, respecting patient rights and cultural backgrounds. Providing patient education, monitoring health needs, and coordinating community resources. Reporting any changes in patient condition to physicians and supervisors. Participating in team meetings, case conferences, and quality improvement activities. Promoting patient empowerment and quality of life, ensuring respectful and non-judgmental interactions. Adhering to policies, including HIPAA, and maintaining licensure and professional requirements. Documenting interactions proficiently in EMR. Assisting with discharge planning and post-discharge care training. Performing other duties as assigned. Knowledge, Skills, and Abilities: Proficiency in documentation and EMR use. Strong analytical, critical thinking, and decision-making skills. Excellent communication, conflict resolution, and people skills. Integrity and compassion; bilingual in Spanish and English preferred. Minimum Education and Experience: Current RN license in Florida (bachelor’s degree). At least two years of nursing experience, preferably in home health or hospital settings. Knowledge of adult chronic disease management and clinical assessment skills. Strong organizational and multitasking abilities.
